ufs: snapshot of UFS controller & PHY driver

This is a snapshot of UFS driver (host controller and PHY) taken as of
msm-4.9 commit <2e98d6c8> ("Merge "u_ether: Handle memory allocation
failure case on tx path"").

Snapshot is taken of following files:
	drivers/scsi/ufs/*
	drivers/phy/phy-qcom-ufs*
	Documentation/devicetree/bindings/ufs/ufshcd-pltfrm.txt

Fixed some of the minor compilation errrors after taking the snapshot.
