isci: uplevel request infrastructure



* Consolidate tiny header files
* Move files out of core/ (drop core/scic_sds_ prefix)
* Merge core/scic_sds_request.[ch] into request.[ch]
* Cleanup request.c namespace (clean forward declarations and global
  namespace pollution)
