CRIS: Update cpu_possible_map and raw_smp_processor_id in smp.h header.

- Change name of __smp_processor_id to raw_smp_processor_id.
- cpu_possible_map is no longer a define for phys_cpu_present_map,
  it is now a cpumask_t.
