ARM: dts: msm: modify VDD_APCC CPR voltage margins for msm8996v3

Characterization has shown that additional VDD_APCC CPR open-loop
and closed-loop voltage margin is required at some power cluster,
performance cluster, and CBF clock frequencies.  Increase the
voltage margin for these frequencies accordingly.

Also add optimized voltage adjustments for MSM8996v3 parts which
have APCC CPR revision fuse equal to 2 or 3.
