msm: clock-8974: Register hdmi clocks in clk_lookup table

HDMI divider clocks needs to be handedoff so that their max_div
and min_div fields can be correctly filled in. Otherwise, the
clk_set_rate will fail on them. Add these clocks in clk_lookup
table to make sure they are handedoff.
