kvm: remove KVM_MMIO_SIZE 



After f78146b0, "KVM: Fix page-crossing MMIO", and
87da7e66, "KVM: x86: fix vcpu->mmio_fragments overflow",
actually KVM_MMIO_SIZE is gone.
