usb: phy: Add snapshot of USB QUSB PHY driver 



This change add USB QUSB PHY driver used on QTI MSM platforms.
This snapshot is taken as of msm-4.19 commit b82cc3a4ac60
("usb: phy: qusb: Do not power down PHY if EUD is enabled").
