ARM: dts: qcom: update modem cooling devices for LITO

Add new wlan modem QMI cooling device and rename few modem
cooling devices name for LITO.

Below are the name changes,
"modem_proc" to "modem_tj",
"modem_skin#" to "modem_mmw_skin#".
