IB/isert: Introduce isert_map/unmap_data_buf



export map/unmap data buffer to a routine that may
be used in various places in the code and keep the
mapping data in a designated descriptor. Also, let
isert_fast_reg_mr to decide weather to use global
MR or do fast registration.

This commit does not change any functionality.

(Fix context change for v3.14-rc6 code - nab)
